Questionnaires on Embodia - Sharing questionnaire groups

To share a questionnaire group with your patient, you must first create one. Learn to create a questionnaire group in our help article, Creating questionnaire groups.
 

To share a questionnaire group with your patient go to My patients, and select your patient. From the patient profile, go to Progress >  All questionnaires tab > Share a questionnaire > Questionnaire group:

Select your questionnaire group from the drop-down menu and click Submit to share:

 You will see the following notification letting you know that the questionnaires have been shared with your patient.
