Practice Management on Embodia - Part 1: Charting - Connecting a completed questionnaire to a chart entry

If you use the questionnaires feature on Embodia to send your patients outcome measure, surveys or intake forms, you can connect a completed questionnaire to a chart entry. To do so, from the chart entry page, click on the Other actions button, and then on Connect questionnaire.

In the pop-up form, select the completed Questionnaire and the Display behaviour. You can choose to display either the full questionnaire, including all answers, or only the name and the score of the questionnaire.

Then click on Submit. The selected questionnaire will be added above the chart items in the chart entry.
